About Us
Chandler Care Givers
is a Not For Profit, Christ centered, information, referral, and encouragement ministry for caregivers. We provide information and reference material for the caregiver. We help you find the information you need to care for your loved one.
We know that caring for an ill or incapacitated loved one can be a full time job. We believe we can help. Having experienced the frustration, confusion and stress of being a caregiver we seek to reach out to those who need assistance finding the services, information and/or support for their loved one.
We are a source of information and encouragement. We provide reference materials at no charge and share links to some companies that serve the Chandler & Mesa AZ areas. This information is for your convenience and we offer no endorsements. We further advise you to check out any business with the proper oversight authorities.

This site, http://www.medicare.gov/caregivers/ contains helpful information for those who are responsible for the health and well-being of an elderly spouse, parent, or friend. The site explains the enrollment process for all of Medicare's different parts and instruct you on how to check whether a drug or device is covered by Medicare, how to file a claim with Medicare, and how to make an appeal. In addition, there is information on nursing homes and there are alternativesand how to pay for themand advice on planning for long-term care. Neighbors Who Care - A local group Providing assistance with transportation, dinner delivery, caregiver relief, even minor repairs. Neighbors Who Care also offers services like: shopping and errands, reassurance phone calls, visitations and assistance with business affairs.
Evercare provides programs for people in need of Hospice and Palliative Care.
Other services available include:
- Care & attention from an Evercare Hospice or Palliative Care Physician,
- Coorrdinate with the patient's personal physician,
- On-call staff available 24/7, Calls returned within 15 minutes.
- Pain management and symptom control,
- Psychological and spiritual support,
- Family support and education including grief support services for a minimum of 13 months,
- Assistance navigating the health care system and Medicare and medicaid certification.

Arizona Division of Aging and Adult Services - The Arizona Department of economic security is very much interested in the aging and their welfare.

What kind of care is available to me in my home?
Government subsidized Home and Community Based Services (HCBS) are available under the Arizona Long Term Care
System (ALTCS, part of the State's Medicaid program). ALTCS provides services in a person's home, and are a possible
alternative to institutional care.
Individuals who do not meet the ALTCS financial and medical eligibility requirements, may be eligible for Non-Medical Home and Community Based Services through the Arizona Department of Economic Security (DES), Aging and Adult Administration. This case managed system of care offers an array of services designed to assist aging and vulnerable individuals to remain and live independently in their home and community. Services provided through contracted agencies include:
The HCBS services available through ALTCS are:
- Adult Day Care - Includes planned care supervision and activities, personal care, training, personal living skills, meals and health monitoring in a group setting for a portion of each day.
- Attendant Care - Includes assistance with activities of daily life such as dressing, bathing, eating and mobility. This service is typically provided daily or several times a week.
- Home Health Services - Includes nursing services, home health aide services, occupational therapy, physical therapy, respiratory therapy and speech therapy.
- Personal Care - Includes assistance with activities of daily life such as dressing, bathing, eating and mobility, which is typically provided once a week, or every-other-week.
- Homemaker Services - Assistance with activities such as cleaning, shopping, meal preparation and laundry.
- Home Delivered Meals - Typically include lunch delivered to a person's home, Monday through Friday.
- Hospice Care - Supportive care and counseling for terminally ill members and their families and caregivers.
- Respite Care - Short-term or intermittent care and supervision to provide rest and relief for the family member caring for a person who is elderly or disabled.
- Transportation - Provided to and from approved health care services.

What resources are available to family caregivers?
Eighty percent of all caregiving is provided by family caregivers. A variety of services are available to caregivers
through the Arizona Department of Economic Security (DES), Family Caregiver Support Program. The program
provides information and services that assist family members or others in caring for an elderly relative in the home.
The program also serves grandparents and other older relatives who provide care to children under age 19. There
are five basic services in the Family Caregiver Support Program: Information; Assistance; Individual Counseling, Support
Groups, and Caregiver Training; Respite Care; and Supplemental Services. Additional information may be found at the
